They lack the shoulder pylons which are implied to primarily act as Eva Restraints, but also as weapon storage and sky-crane style rockets. But as for the mouth, Adam was not shown with one and my best guess is that it was a story-driven artistic decision that was very successful in creeping me out. More allusions can be pointed at how they also have wings. This is extremely odd for Evas, so why are they made like this? The only clue I've come up with is that the MPEs "might" actually resemble Adam closer than the other Eva types because he has no clear distinction between torso, neck, and head from what little has been shown. Their neck and head are one whole, they lack any eyes, and that in/famously uncovered mouth. Looking at the MPEs as a whole, here are some questions and tidbits I have after rewatching NGE+EoE and doing 10+ hrs of researching Evangelion lore available online: All the Evas resemble the human shape quite closely and the most deviant ones would be Unit 00 having maybe just 1 eye (or 2, arranged vertically) and Unit 02 having 4 eyes along with the non-red blood (most likely just to contrast its red scheme).
